- Health Inventory – Part I and Part II (doctor’s signature required on Part II)

- Medication Authorization – If your child does not take regular medications, please fill in the child’s name and write ‘None’ or ‘NA’ on the form and send it in.
- Immunization Form – The Health Department requires this form to be on file at school before a student can begin attending classes.

If your student is coming directly from a homeschool environment, please use the following alternative list of required forms.
- Online Application: Select the appropriate application for the grade the student will be entering.
- Pastoral Questionnaire
- Immunization Form – The Health Department requires this form to be on file at school before a student can begin attending classes.
- Copy of Birth Certificate
- Enrollment Agreement
- A Parent-Educator Evaluation
- Elementary Parent-Educator Evaluation (for students entering 1-5 grade)
- Upper School Parent-Educator Evaluation (for students entering grades 6-12)
- Non-Parent Teacher or Leader Evaluation
- This form is to be filled out by another adult of your choice (non-parent) who has spent significant time with your student in a teaching or leadership context, and who can evaluate the student in a group context. Options include a co-op teacher, activity group/club leader, etc.
- Documentation/records regarding your child’s homeschool studies.
- Records should be submitted in a printed format that our admissions committee can review prior to your admissions interview, to give CLS a sense of your student’s academic placement. Suggested items include:
- Examples of recent student work in both Math and English.
- Curriculum overviews/outlines for the last two years; to help us assess placement (Overviews/Outlines can be printed directly from a publisher, or created by the parent if you are following a self-designed curriculum.)
- Records should be submitted in a printed format that our admissions committee can review prior to your admissions interview, to give CLS a sense of your student’s academic placement. Suggested items include:
Report cards and/or transcripts from any homeschooling parent organization or co-op (for example, FSP, MCI, Cedarbrook)
- Online Application: Select the appropriate application for the grade the student will be entering.
- Toefl Test (for students who do not come from English-speaking countries)
- English Teacher Recommendation – for Non-Native-English-Speaking Internationals
- International Record Release Authorization: This form is to be given to your current school. If there are no bilingual administrators available, the student and his or her parents must secure the services of an English translator to help an administrator complete this form.
- Immunization Form: Immunizations must be translated to English before being sent. The Health Department requires this form to be on file at school before a student can begin attending classes.
- Principal/Director Recommendation: If your current school’s Principal/Director is not bilingual, the student and his or her parents must secure the services of an English translator to help an administrator complete this form.
- International Character Recommendation: If the person filling out this recommendation is not bilingual, the student and his or her parents must secure the services of an English translator to help an administrator complete this form.
